A prop Pip-boy 3000 from Fallout 3. Made it a few years ago and it's still my favorite prop, save for the fact that I wish I had made it with some sturdier materials. It's almost entirely made of cardboard and stryofoam with a clever paintjob so it's a lot less durable than it looks. It's got a panel of LEDs inside behind the paper screen for backlighting (runs on two D batteries). My favorite bits are the Geiger counter which was hand drawn with a really tiny pen, and the blood/grime on the screen which was just plain fun to do.
A rebuild is in the works, possibly involving parts being built roughly like this one, but then molded and cast in plastic/resin.
